Application Procedure for Chettinad Hospital and Research Institute Medical Entrance Exam - 2015

For M.D / M.S – 2015

Application Fee:

Application forms can be obtained from the office of the Registrar, Chettinad Academy of Research and Education on payment of 2,000/- ( Rupees Two thousand only ).

Last date for receipt of filled in application form is 10.01.2015 ( Saturday )

If the application form is downloaded from the university website a demand draft for 2,000/- ( Rupees Two thousand only ) drawn in favor of “Chettinad Academy of Research and Education” payable at Chennai to be enclosed.

Application form should be filled in carefully and legibly by the candidate in his / her own handwriting.

Furnishing of false information or suppression of material information in the form will make the application treated as cancelled.

The University reserves the right to change any of the conditions enumerated periodically or for the purpose of complying with any of the regulations of the Statutory Councils.

The Institute shall not be held responsible for any postal delay, loss in transit or for incorrect address given by the candidates.

Certificates to be enclosed along with the application form:

Photo copies of the following certificates self attested should be enclosed along with the application form for Entrance Examination

a. Mark sheet of HSC Examination

b. M.B.B.S. Degree Certificate from the concerned University / Provisional Pass Certificate I and II

c. Mark sheets of the University Examination for the whole of the M.B.B.S. course

d. Attempt Certificate ( First year to Final year ) wherever applicable

e. Community Certificate…… for the purpose of statistics only

f. Internship Certificate ( C.R.R.I. ) Completed on or before 31.03.2015 / Self declaration for completion of C.R.R.I

g. Permanent Registration Certificate issued by Medical Council of India or State Medical Council

h. Transfer Certificate

i. Migration Certificate wherever applicable

j. Conduct Certificate issued by the Head of the Institution last studied

If any candidate has not completed internship at the time of submission of application he / she should enclose a certificate obtained from the head of the institution in which he / she is undergoing the internship certifying that he / she is likely to complete the internship on or before 31.03.2015, failing which the admission is liable for cancellation.

Two identical and recently taken passport size color photographs are required. Candidate should affix the photograph in the space provided
