Application  procedure for MMU M.Sc Entrance Examination – 2014

Admission to 85% open seats of M. Sc. Nursing course will be made on the basis of merit of the Entrance Test.

Admission to 15% NRI seats will be advertised separately. The candidates for NRI seats are not required to appear in the Entrance Test.

A candidate seeking admission to the M. Sc (Nursing) course is required to send his or her application on the prescribed form appended at the end of this prospectus.

Before filling up the Application Form, the candidate is required to go through the prospectus carefully and acquaint himself / herself with all the requirements with regard to the filling-in of the Application Form and check his/her eligibility.

Application Form should be filled-in in English only.

The candidate shall fill-in the Application Form in his / her own handwriting in block letters written neatly and legibly with black ink within the prescribed boxes.

Overwriting  / cutting / erasing in the Application Form may lead to rejection of the Application and should be avoided. Any error arising on this account shall be the responsibility of the candidate. 

The candidate should write his / her complete address in the Application Form for further correspondence. It will be the responsibilities of the candidate to ensure that he / she gives his/her correct address. The University or the College shall not be held responsible for any delay or loss of any document/correspondence in transit or for an incorrect address given by the applicant.

Application Form duly completed must be accompanied by one self- addressed stamped envelope (10” ××5” ) with postal stamps of Rs. 50/- affixed on the envelope.

The last date for receipt of Application Forms, duly completed in all respects, is 15-05-2014 (Upto 4 pm).

Candidate should write his/her name, father's name and mother's name in capital letters as given in his/her Matriculation/High School Certificate of the Board. Only one letter should be written in one box. One box should be left blank between each part of the name.

Date of birth: Enter the date, month and year of birth as per English calendar and as recorded in the High School/I.C.S.E./Class X or its equivalent certificate.

Photograph: Affix a recent (not taken before 01-07-2013) high contrast photograph in the space earmarked for it on the Application Form.

Accessories like caps, hats or dark spectacles are not permitted in the photograph. Polaroid camera photos are not acceptable. The photograph should not be defaced. The photograph should be firmly affixed to the Application Form with adhesive (gum/fevicol) and should not be pinned or stapled. The candidature of the candidates not complying with these instructions or with unclear photographs is liable to be rejected.

Every candidate is required to submit only One Application Form. If a candidate submits more than one Application Form, his/her candidature against all the applications is liable to be cancelled.

Furnishing of false information or suppression of any information would disqualify the candidate from the admission/entrance test and cancellation of admission if already admitted. If considered necessary, legal action may also be taken against such a candidate in addition to forfeiture of the fee already paid.

The Application Form must be accompanied by the following certificates / documents duly attested:

a) Matriculation or equivalent Certificate ( as proof of date of birth )

b) Certificate of Qualifying Examination.

c) Certificate of Registered Nurse and Registered Midwife

d) Certificate of Work Experience.

e) Character Certificate reporting on the inter personal /social behavioral pattern of the candidate issued by the Institution last attended by him/her.

f) SC / ST Certificate having been issued not prior to 1.7. 2013 for claiming relaxation of 5% marks in eligibility for admission to M.Sc. Nursing.

g) Migration Certificate: In case the Migration Certificates is not readily available, it may be submitted within 15 days of admission failing which the concerned student will not be allowed to attend the classes and his admission will be cancelled without any further notice and without any refund of fee paid by him/her.

h) Admit Cards appended at the end of this Prospectus, duly filled-in in triplicate with a ticket size photograph on each.

i) Photographs to be affixed on the Application Form, Admit Cards and an additional Photograph to be enclosed.
