NEIGRIHMS – B.Sc Medical Entrance Examination Application Procedure – 2014

Option 1:-

 Candidates collecting Prospectus cum Application Form from NEIGRIHMS counter may collect the fee Payment Challan from NEIGRIHMS counter located at Director’s Block ( Ground Floor) or from Bank of Baroda, Mawdiangdiang Branch, located inside NEIGRIHMS campus and deposit an amount of Rs. 500/- ( Rupees Five Hundred Only ) in any branch of Bank of Baroda across India.

Option 2:- Candidates downloading Application Form and Fee Payment Challan from the Institute’s website at www.neigrihms .nic.in may deposit in any branches of Bank of Baroda and Submit / send the filled in Application From along with the NEIGRIHMS copy of the Challan in Original to “Examination Cell, NEIGRIHMS, Director’s Block, P. O. – Mawdiangdiang, Shillong – 793018, Meghalaya”

Option 3:- Candidates downloading Application Form from the Institute’s website ( www.neigrihms .nic.in) from where Bank of Baroda Branch is not available may deposit the fee through NEFT from any Nationalized Bank in the Institute’s account by filling up the following details:

1. Name of the Account : “PRINCIPAL NURSING COLLEGE OF NEIGRIHMS”

2. Account Number : “30270200000002”

3. IFSC: “BARB0MAWDIA” below and submit /send the Filled in Application Form along with the Original NEFT Counterfoil ( Please retain with you a Xerox copy of NEFT Counterfoil ) to “Examination Cell, NEIGRIHMS, Director’s Block, P. O. – Mawdiangdiang, Shillong – 793018, Meghalaya” Please note that NEFT UTR number must be clearly written on the counterfoil from the fee accepting Bank.

Submission of Application Forms:

1. Duly filled in application form addressed to “Examination cell, NEIGRIHMS, Director’s Block, P. O. – Mawdiangdiang, Shillong 793018, Meghalaya” should be submitted at the Drop Box of Examination Cell, NEIGRIHMS placed outside the Examination Cell, Director’s Block, NEIGRIHMS. Duly filled in application form can also be sent by post addressed to “Examination cell, NEIGRIHMS, Director’s Block, P. O. – Mawdiangdiang, Shillong 793018, Meghalaya”.

2. (Kindly note that a challan Transaction ID number (Original) should be enclosed while submitting the Application Form downloaded from the internet)

Criteria for Rejection Of Application Forms:

1. Candidates who failed to fulfill the age criterion

2. Candidates who failed to obtain the minimum stipulated marks in the qualifying examination    ( Higher Secondary / 10 + 2 )

3. Faulty submission and incomplete details ( signature, thumb impression, etc. ) in the forms or non - receipt of payment will be summarily rejected.

Certificates and Documents to Be Submitted

Attested copies of the following Certificates /Documents should be submitted along with the Application Form to the Examination Cell, Director’s Block, NEIGRIHMS, Mawdiangdiang, Shillong-793018, and Meghalaya.

Age proof:

 High School Certificate / Secondary School Certificate / Admit Card of 10th or 10+2 Exams issued by the Board / University showing the date of birth / Birth Certificate of the Applicant issued by the competent Government authority.

Certificates:

Pass Certificate / Provisional Pass Certificate in case the candidates already pass 10+2 Examination.

OR

Admit Card / appearing certificate ( from Principal of School or College ) of 10+2 Examination for those Candidates who are waiting for the declaration of result.

Mark sheets:

i. Clearly readable mark sheet of the 10+2 examination showing the marks secured by the applicants.

ii. In case, a candidate is waiting for result of 10+ 2, an ADMIT CARD of the said examination should be submitted along with the application form. However, original mark-sheet should be produced at the time of Counseling if qualified.

Employed Candidates:

In case of employed candidates, he / she must forward their application through proper channel. In case delay is anticipated in the transmission of the application form, an advanced xeroxed copy of the same, complete in all respects must reach the In-charge, Examination Cell, Director’s Block, NEIGRIHMS, Mawdiangdiang, Shillong-793018, Meghalaya, before the closing date.

The application Through Proper Channel should reach the In-charge, Examination Cell, Director’s Block, NEIGRIHMS, Mawdiangdiang, Shillong-793018, Meghalaya., within ten (10) days after the last date for the receipt of applications.

Photographs:

Two (2) recent clear colour passport size photograph All claims made in the application must be supported with documentary proof.
