Prospectus cum Application form is available and downloadable w.e.f. 16th June, 2014 to 14th July, 2014 from the NEIGRIHMS website at www.neigrihms.nic.in. Candidates downloading the Prospectus cum Application form may refer to the procedure of payment available at clause 7. vi below.

Note:

No request for sending the Prospectus cum Application form by post will be entertained

Submission Of Application Form And Procedure Of Payment:

i. Before applying, candidates are advised to go through the Prospectus and the Instructions given therein carefully and then fill in the application form given along with the prospectus.

ii. Application forms can be obtained as above in Clause 6 above

iii. Duly filled in Application forms can be submitted at the “Drop Box of Examination Cell” placed outside the Examination Cell,

Shillong – 793018 from 16th June, 2014 to 14th July, 2014 between 10.00 am to 4.30 pm  ( Monday to Friday except the Central Government Holidays ) .

Application Forms must be addressed to “Examination Cell, NEIGRIHMS, P.O. – Mawdiangdiang, Shillong – 793018” supercribing the envelope as “Application for DM in Cardiology Entrance of NEIGRIHMS”.

iv. Candidates must send their duly filled in Application Forms by post addressed to “Examination Cell, NEIGRIHMS, P.O. – Mawdiangdiang, Shillong – 793018” superscribing the envelope as “Application for DM in Cardiology Entrance of NEIGRIHMS”. Application Forms much reach NEIGRIHMS on or before 14th July, 2014  ( 4.30 pm ) .

v. However, candidates must send a scanned copy of their duly filled and signed application form as attachment in email the address - neigrihmsexamination@gmail.com as advance copy. Please do not compress the scanned image. However, candidates must pay the Examination Fees through any of the options as indicated in Clause 7. vi. below. Candidates must attach the scanned copy of the original NEIGRIHMS Challan / Photostat copy of NEFT transaction counterfoil along with the Application Form.

NEIGRIHMS will not take any responsibility for non receipt or late receipt of application forms

vi. Procedure of payment:

Candidates can download the Fee Payment Challan available and downloadable along with the Prospectus cum Application Form from the Institute website www.neigrihms.nic.in. Fees payment can be made by one of the following options:

a)  Option 1:

 (Recommended )  Candidates can deposit the Fees of Rs. 3000 / -  ( Rupees Three Thousand Only )  through the downloaded Challan at any Branch of Bank of Baroda in  India. Candidates submitting the dully filled in Application Form, either personally or by post shall have to enclose the original NEIGRIHMS copy of the Challan in original. Candidates are advised to retain the Candidate`s copy of the Challan.

b)  Option 2:

Candidates may also deposit the Application Fee through National Electronic Funds Transfer  ( NEFT )  from any nationalized bank in India at the Institute Account: 

(1). Name of the Account: “NEIGRIHMS MBBS ACCOUNT” 

(2). Account Number: “30270200000005” 

(3). IFSC: “BARB0MAWDIA”.

Duly filled in form must enclose  the original NEFT Counterfoil  ( Please retain the photo copy of the counterfoil )  and must be submitted  / sent addressed to “Examination Cell, NEIGRIHMS, P.O. –Mawdiangdiang, Shillong – 793018” superscribing the envelope as “Application for  DM in Cardiology Entrance of NEIGRIHMS”.

Note:

NEFT UTR number must be clearly written on the counterfoil form by the accepting Bank. NEIGRIHMS will not take any responsibility for late receipt / non-receipt of the application form. An application number will be allotted to the each application form once the Application form is received by the Institute.

Candidates should take care to write their complete and correct correspondence address in the Application Form. However, candidates should retain a photocopy of the Application Form along with the identical photograph.

Incomplete applications in any respects will be summarily rejected and no correspondence / queries will be entertained in this regard.

All particulars in the Application Form must be written carefully and legibly in candidates own handwriting.

Candidates are required to put their signatures and the left thumb impression in the particular column provided in the Application form.
