Pramukhswami Medical College Pre-PG Medical Entrance Examination Application Procedure-2015:

For MD / MS and Diploma PG Courses:

Application form is to be downloaded from the website and filled for submission.

The completed application form with documents and the necessary application fee for the categories of choice to be submitted at the office before the last date of application

The application fee is to be paid in cash or demand draft drawn in favor of “Charutar Arogya Mandal”, Payable at Anand on any nationalized bank. ( Cheques will not be accepted )

Admission committee is not responsible for any loss / delay / damage which might occur during Postal or Courier delivery Forms will not be accepted after the due date 10th Jan 2014

Application form can be obtained from PG cell by paying the application fee of Rs 3000/-in cash or demand draft drawn in favor of Charutar Arogya Mandal, Payable at Anand on any nationalized bank

Applications with all enclosures should reach, on or before the Last date 09th January, 2015 at the following address:

Documents are required to submit along with Application Form:

Forms downloaded from website should be printed on A4 size Paper.

Any Other Size Will Not Be Accepted.

Do not fold the application form

Do not staple photograph to the form, paste it in the space provided.

Do not staple the certificates to the form, use ‘U’ clip.

• Application Form should be filled as per guidelines.
• DD in favor of “CHARUTAR AROGYA MANDAL”, payable at ANAND on any nationalized bank.
• Cheques will not be accepted. Cash will be accepted for those form submitted in person at the office.
• Birth Certificate / School Leaving certificate
• A photo ID proof ( Driving License, Passport etc.)
• Copies of mark sheets for the 1st, 2nd and 3rd ( Part – I and Part – II ) MBBS issued by the University with all attempts. Certificates issued by other authorities will not be acceptable.
• Internship completion certificate issued by the college.
• Degree Certificate ( Provisional / Permanent ) and Passing certificate issued by the University.
• Attempt Certificate issued by the college.
• Medical Council Registration Certificate / State medical council registration certificate ( Permanent or temporary ).
