Sri Guru Ram Das Institute of Medical Sciences & Research ( SGRDIMSR ) PG Medical Entrance Exam - 2015 – PG Application Procedure

For NRI Quota MD / MS / MDS – 2015

Online Application Form is available for admission to MD / MS / MDS ( NRI Seats ) in Sri Guru Ram Das Institute of Medical / Dental Sciences & Research, Sri Amritsar respectively.

Examination Fee and Cost of Prospectus cum Information Bulletin ( For Sikh Minority Quota ):- Fee payable along with Online Application Form Rs. 9500-00 ( in shape of Bank Challan )

Last date for receipt of completed Application Form is 05.3.2015 by 3.00 p.m. However, all the NRI candidates will have to qualify “PG Entrance Test” which will be conducted by the institute ( only for NRI Candidates ) itself on 11.5.15 at 11:00 am in this institute’s premises.

For Sikh Minority Quota ( SMQ ) MD / MS / MDS – 2015

Online applications are invited from eligible Untrimmed Sikh candidates for admission to MD / MS / MDS courses under Sikh Minority Quota seats for the Session 2015-2016.

The above fee is inclusive of Sikh history & religion examination fee and information bulletin. Please note that information bulletin will not be sold separately. Information bulletin shall be available at Institute’s official website

Results of AIPGMEE-2015 conducted by National Board of Examinations for admission to all India 50% Quota of MD / MS / Diploma seats shall be utilized for admission to MD / MS Courses ( Sikh Minority Quota seats ).

For admission to MDS course under the Sikh Minority Quota seats the result of similar competitive examination conducted by competent authority ( authorized by Govt. of India / Hon’ble Court ) shall be utilized.

However, all the candidates will have to qualify “Sikh History & Religion” examination, which will be conducted by the institute itself on 14-1-2015 at 11:00 am in this institute’s premises. No further advertisement will be given in the newspaper in this regard, so stay updated and kindly visit official website regularly.

Visit the institute official website and click on “Online Admission, Registration / Login” OR Visit directly to the online admission portal. No other means / mode of application ( through post, email, fax, deposit of CV etc. ) will be accepted.

Candidates are advised to complete the formalities well in time to avoid last date rush on web server.

Firstly ensure that you have a valid personal email ID. In case, a candidate does not have a valid personal email ID, he / she should create his / her new email ID before applying Online.

You should keep checking the inbox or even the spam box of this email id regularly during the admission process as the institute may send intimations for written test etc. through this.

Create your log in account by filling your basic details.

Now login to your account and download & print Bank Challan bearing fee type and your basic details. You can deposit the fee amount in any branch of State Bank of India after 2 days of registration of your login account at Online Admission Portal.

Kindly check all the three leaflets very carefully for Reference No., Fee Type and your personal details.

It is important to note that the requisite fee can only be deposited in any of State Bank of India branches as the institute has only nominated State Bank of India for Collection of Fee. Fee paid in any other bank or in any other format ( Cheque, Postal Order, Demand Draft etc. ) will not be accepted.

The bank, after getting fee amount along with bank charges, will keep the bank's copy of challan and return other two copies i.e. institute's copy and candidate's copy back to the candidate.

Kindly note that the bank will write the Journal No., Branch Code, Date of Deposit on bank challan after accepting the fee amount

After 48 hours of depositing the fee in bank, candidate will login again in the Online Admission Portal and required to fill the Journal No ( mentioned in Bank Challan ) to proceed with filling the application form.

Candidate is advised to fill all the column of On-Line Application Form very carefully as once submitted, the candidate cannot edit his / her application form.

After finalizing the on-line application, the candidate is required to print at least 2 copies of the form and get them signed.

Candidate has to send one of the application form's hard copy along with the supporting document and institute's copy of bank challan to the institute either by hand or through post on or before 20.12.2014 by 03:00 pm.

The candidate is also required to download his / her Admit Card and get it printed ( minimum 2 copies to be safer) for the entry in Examination Centre ( Sikh History & Religion Exam ) and in the Counseling Hall. Entry without Admit card is not allowed in any case.

Candidate may note that there is no option for submitting the form other than the online mode. Offline applications will not be accepted.

Online Applications for admission to the examination should be completed by the prescribed cutoff date ( see “Important Dates” on cover page ).

Candidate is required to upload his / her signature and latest passport size photograph with following specifications on the Application Form & Admit Card.

A photograph of minimum 35X45 mm with at least 75% coverage of face & head of the candidate

A caption indicating name of the candidate and date of taking photograph should be there at the bottom of the photo.

The Photograph needs to display full front view of the face. Please look directly into the camera with a neutral expression.

Please avoid photograph with reflection or shadow on the face with red eyes.

Candidate is required to submit a print copy of online application form to the institute. The print copy of online application form must reach the below address

Before the closing date and time i.e. 20.12.2014 up to 03:00 pm either by hand or by Registered post / speed post.  In any case, if the application is delayed in transit, the responsibility will be entirely of the candidate.

Note: Sending admission form through a courier service is absolutely not advised because no courier service delivers at the institute office.

Sending the admission form through speed post should also be avoided as some times serious delay is likely to occur, for which the candidate himself / herself shall be responsible.

An application form which does not reach the office of the Registrar-Examinations within the prescribed date and time shall be summarily rejected. In such a situation, the examination fee shall not be refunded.
