Sumandeep Vidyapeeth University, Vadodara was established in 1981 as K.M.Shah Charitable Trust. The Trust initially engaged in providing Dental Healthcare services. SVU 2021 PG - Application Procedure

Sumandeep Vidyapeeth ( SVU ) University Application Procedure for Post Graduate Course – 2015 :

A candidate desirous of appearing for All India Post Graduate Medical and Dental Entrance Test of this University will be required to submit application in the prescribed form along with the attested copies of the documents in support of the application.

Application Fee

The application form and brochure can be obtained from university office or by downloading till 18th Jan 2014 from university website www.sumandeepuniversity.co.in, and is to be submitted along with demand draft of Rs.2500/- ( form fee plus test fee )

The application form downloaded after 17th Jan 2015 is to be submitted along with demand draft ofRs.3000/- ( form fee plus test fee plus late fee Rs. 500)

The fee is payable by a demand draft, drawn in favor of Sumandeep Vidyapeeth of nationalized / schedule bank, payable at Vadodara

The candidate should write his / her name, address and mobile number on the reverse side of the demand draft

The last date of receipt of completed application form along with requisite documents and entrance test fee at the office of Registrar, Sumandeep Vidyapeeth Piparia on 17th Jan 2015, upto 02.00 p.m. and with late fee of Rs.500/- on 21st Jan 2015, 06.00 p.m.

Documents are needed to submit for SVU University -2015 :

The attested copies of the following documents must be submitted along with application form for Entrance Test and originals should be submitted at the time of counselling.

• Birth Certificate / School Leaving Certificate.

• First M.B.B.S. / B.D.S. Examination mark Statement

• Second M.B.B.S. / B.D.S. Examination marks Statement

• Third M.B.B.S. ( Part i & ii ) /B.D.S. Examination Marks Statement

• Rotating Internship Training Completion Certificate

• A letter indicating likely date of completion of Rotating internship Training from the concern Deam/ principal of the college

• Registration Certificate issued by State or Central Council

• Bonafide certificate from the Dean / Principal of the College
